  1. This form (or its paper equivalent) must be filled out and returned to Audio Precision before your instrument is returned for service or repair. AP will contact you to arrange a shipping date.

  2. Customers must establish the method of payment prior to sending their instrument in for repair. If the work falls under warranty, you will not be billed. If you have a current open account with Audio Precision, please provide a purchase order number in the form below. New customers will need to pay by credit card.

Shipping Box
If you have to return your instrument for repair, it must be sent in a proper box with proper packaging material. An inadequate package will not protect the instrument from shipping damage and repair costs for physical damage can be high. If you do not have the original shipping material you may request an empty box with all the appropriate shipping material and instructions. Please read consequences of improper packaging and view instructions for packaging your system.

I do not have adequate packaging material to get the unit from my facility to the Factory. Please send an empty box and packaging for me to use (send to the above shipping address). I understand a $30 charge will be added to my repair costs.
